要旨　患者は63歳，女性．呼吸苦を主訴に当院を受診し，心臓左房腫瘍を発見し，心臓血管外科にて完全摘除し，病理組織学的にMFH（malignant fibrous histiocytoma）と診断した．術後4年目に心窩部不快感で当科を受診し，上部消化管内視鏡にて十二指腸主乳頭の前壁側に10mm大の扁平な粘膜下腫瘍を認め，同腫瘍からの生検にてMFHの十二指腸転移と診断した．患者・本人の希望により経過観察し，術後に発症していた心不全・呼吸不全の悪化により，術後6年1か月に永眠した．剖検で，十二指腸腫瘍は70mm大に増大し，形態は有茎性の粘膜下腫瘍へと変化していた．心臓原発MFHの十二指腸転移の1例を，文献的考察を加えて報告する．

　A 63-year-old woman was referred to our hospital complaining of respiratory discomfort. Examinations revealed a left atrial tumor, and it was resected completely. Microscopic findings of the resected specimen showed MFH（malignant fibrous histiocytoma）. 4 years after surgery, she complained about epigastric discomfort. EGD（esophagogastroduodenoscopy）revealed a submucosal tumor of the duodenum, and pathology comfirmed metastatic MFH. She died because of heart failure and respiratory failure 6 years after surgery.
