 The pathological diagnosis of Barrett esophagus presents many challenges. It is not always easy to prove histologically that it is the esophagus that is affected, and identification of the esophagogastric junction must be done comprehensively based on various findings. Collaboration between the clinician and pathologist from the time the specimen is submitted is important for a proper diagnosis. Since Barrett adenocarcinoma is an inflammatory carcinoma, it should be carefully differentiated from reactive changes, and immunohistochemistry should be added if necessary. The diagnosis of progression is often accompanied by complications of the muscularis mucosae, and improving interdiagnostic concordance is an issue for the future. In addition, the method of describing esophagogastric junction carcinoma in the standard regulations needs to be improved.
