活性型ビタミンＤ治療の進歩と課題

New Development in Osteoporosis Treatment. Recent progress and problem of treatment with active vitamin D.

　従来使用されてきたアルファカルシドールやカルシトリオールに加え，エルデカルシトールが本邦で骨粗鬆症に対し使用可能となった。エルデカルシトールは，アルファカルシドールに比較し，より強い椎体骨折抑制効果を示すとのエビデンスが確立された。エルデカルシトールは主に，骨吸収の抑制により骨密度を増加させるものと考えられる。ただしいずれの活性型ビタミンＤ製剤でも，高カルシウム（Ca）尿症や高Ca血症などの有害事象に注意する必要がある。

　Alphacalcidol and calcitriol have been used as active vitamin D drugs for patients with osteoporosis. In addition, eldecalcitol was recently introduced into clinical practice as a novel active vitamin D in Japan for patient with osteoporosis. It has been established that eldecalcitol reduces vertebral fractures more efficiently than alphacalcidol. Eldecalcitol seems to increase bone mineral density mainly by suppression of bone resorption. However, it is necessary to pay attention to adverse events like hypercalciuria and hypercalcemia in all the patients taking active vitamin D drugs.
