 In 2023, 29.1% of the total population of Japan was aged ≧65 years. When considering patients’ anesthesia risk, their biological age should be assessed rather than chronological age. Frailty is a factor in the concept of older biological age. Subjecting a frail elderly patient with a stressful surgery, regardless of it major or minor status, will result in a worse prognosis compared to those of robust patients.

 It is thus of great significance to determine whether or not a patient is frail before surgery in order to provide appropriate interventions. However, the number of geriatricians in Japan is rather small, so that even if a patient is diagnosed as frail preoperatively, the number of hospitals that can provide appropriate interventions may be limited. In addition, because it takes a long time to improve the state of frailty, some oncologic surgeries may not allow sufficient time for intervention. In such cases, the perioperative physicians should discuss with the patient and his/her family whether surgery is still the treatment of choice.
