 Japan continues to experience unprecedented demographic changes with an extremely high proportion of older individuals, which has contributed to a marked increase in the number of older people undergoing surgeries. To properly evaluate the risks of surgical anesthesia in older people, it is important to understand age-related physiological changes. One of the features of age-related physiological changes is impaired homeostatic mechanisms, which is also called ‘homeostenosis.’ Clinicians should be aware that older people often have reduced physiological reserves, even if they do not exhibit any physiological abnormality in the absence of stress. Older patients may thus easily reach a point of no return at which compensation is no longer possible under stressed conditions. In addition to understanding the physiological changes of each organ, evaluations of ‘frailty’ and the use of a comprehensive geriatric assessment(CGA)can contribute to the selection of effective interventions in the perioperative period. Since the degree of homeostatic impairment varies among older people, biological markers that accurately reflect physiological aging are under investigation.
