 Chronic pain in the elderly constitutes a significant global health challenge, particularly in Japan, where 30% of the population is aged 65 or older, and>10% are over the age of 80. The progression of this super-aged society has been accompanied by an increasing prevalence of age-related physiological changes and neurological dysfunctions, many of which contribute to the development and persistence of chronic pain. Although exercise therapy is widely recognized as the primary intervention for chronic pain, its effectiveness in the elderly is often limited, underscoring the need for rehabilitation approaches specifically tailored to age-related challenges. Age-related factors such as sarcopenia, frailty, locomotive syndrome, chronic inflammation, oxidative stress, glycation, and dementia interact to sustain and exacerbate pain. Thus, although exercise therapy remains a key recommendation for pain rehabilitation, tailored exercise prescriptions are essential to address the various challenges encountered by elderly patients. Effective pain management in the elderly requires a personalized rehabilitation approach that is attuned to the distinctive physiological and neurological transformations accompanying the aging process.
