男性に認めた弾性線維腫の3例

Three Cases of Elastic Fibroma in Men

はじめに

　弾性線維腫は1959年にJarviら 1）によって初めて報告された特異な腫瘍性病変で，組織学的に膠原線維の集塊の中に，弾性線維と同様の染色性を示す顆粒状，線維状物質が多量に混在することから命名されたものである。重労働歴のある中高年女性の肩甲骨下方に好発する比較的まれな良性腫瘍であり，身体所見と画像所見から診断が可能である。臨床症状としては腫瘤の自覚のみのことが多いが，疼痛や圧痛，弾発現象を伴うこともあり，このような症状を認める場合は腫瘤の摘出を検討する。本邦では九州・沖縄出身者に報告が多かったことから遺伝的要因が発生にかかわっているとされていたが，最近では小外傷の繰り返しによる弾性線維の反応性過形成であるとの報告が多い 1）～6）。

　今回われわれはまれな男性での症例を3件経験したので報告する。

　Elastic fibroma has been described as a benign tumor that develops in the lower part of the shoulder blade of middle-aged and elderly women. The etiology of elastic fibroma is thought to involve environmental and genetic factors. We encountered three cases of elastic fibroma in men. All three tumors were located in the lower part of the shoulder blade, and elastic fibroma was suspected based on the tumorsʼ location and imaging findings. Because the tumors were growing and symptomatic, surgical resections were performed for all three patients under general anesthesia. We provide the case details of these relatively rare cases of elastic fibroma in males.
