 We describe the cases of nine patients who underwent breast augmentation with implants following the removal of non-absorbable filler polyacrylamide gel. Although polyacrylamide gel had been used widely due to its long-lasting effects and its ability to dissolve with saline, complications (including infections and deformities) led to a ban of its use in breast augmentation by several Japanese medical associations in 2019. Despite the ban, there has been no clear guidance on how to manage patients who have already undergone a breast augmentation that included this polyacrylamide gel. We have conducted the simultaneous removal of polyacrylamide gel and breast augmentation with implants in nine patients at our institution since 2021. All of the surgeries were performed after the absence of infection was confirmed. The successful results achieved for all nine patients indicate that conducting a simultaneous breast augmentation with implants is feasible for patients seeking both gel-filler removal and the maintenance of breast size, provided there are no signs of infection. Key challenges include the difficulty of removing all of the polyacrylamide gel and predicting the implant size after the gelʼs removal. Our experience suggests that this approach is viable for patients with a history of polyacrylamide gel injection, including those with breast cancer, as an alternative to more invasive reconstructions. This report highlights the importance of a careful assessment and post-operative monitoring to ensure favorable outcomes in such cases.
