 Cases of breast reconstruction using silicon breast implants after nipple-sparing mastectomy as a treatment for breast cancer are increasing. The goal of breast reconstruction is to obtain symmetrical breasts, including the nipple-areola complex (NAC). Malposition of the NAC has occurred in some breast reconstructions. We describe cases using a (1) V-Y flap technique, (2) transposition flap technique, or (3) composite graft technique to correct NAC malposition after nipple-sparing mastectomies. The malpositions were successfully corrected with our techniques without leaving any re-displacement or conspicuous scar.
