 Exoscopic surgery is performed while viewing the image captured from above the surgical field. Exoscope systems emerged in the field of neurosurgery and rapidly spread as an alternative to microscopes due to their innovative imaging technology and downsized camera. Exoscope systems have also recently been applied in the field of plastic surgery. We have used the ORBEYE Ⓡ (Sony Olympus Medical Solutions), a 4K-resolution 3D exoscope system, for several plastic surgery procedures, and we report some findings. (1) The system shared the surgical field with all of the personnel in the operating room, which resulted in a smooth procedure. (2) Complicated designs and procedures such as cleft lip and palate surgery were recorded in detail by the system, and the resulting videos have been useful for the preparation and review of the surgical procedures. (3) In narrow and deep surgical fields (e.g., orbital floor fractures and cleft palates), the system enables a surgeon to obtain an ideal view without a burden to his or her neck. Exoscope systems thus have multiple advantages, and their further use in the field of plastic surgery is expected.
