膿瘍形成と治癒の反復に伴う多発線状瘢痕およびヘモジデリン沈着を認めた脾膿瘍の1例

Splenic abscess with multiple linear scarring and hemosiderin deposition due to recurrent abscess formation and healing：a case report

脾膿瘍は比較的まれな疾患であり，通常は免疫の低下した患者や全身感染症のある患者に発症する。今回我々は，基礎疾患のない成人女性に発症し，数年にわたり無治療下で寛解と再燃を反復するという非典型的な経過を示した反復性多発膿瘍の1例を経験した。繰り返される膿瘍と治癒機転により特徴的な瘢痕性変化を呈した本例について，文献的考察を加えて報告する。

We report a case of recurrent splenic abscesses in an otherwise healthy woman in her 50s. She presented with episodes of fever and left-sided abdominal pain occurring every few months over a five-year period. Contrast-enhanced CT and MRI revealed multiple splenic abscesses, irregular splenic contour, and linear scar-like changes with hemosiderin deposition. These imaging findings were histologically confirmed following splenectomy, represent the accumulation of repeated abscess formation and healing. Although atypical for a single episode, these cumulative features are valuable for diagnosing long-term relapsing splenic abscesses.
