EVAR術前の内腸骨動脈塞栓術においてIMPEDE-FXの並列留置が有用であった1例

Parallel deployment of IMPEDE-FX for internal iliac artery embolization prior to EVAR：a case report

腹部・腸骨動脈瘤に対するステントグラフト内挿術（endovascular aortic repair：EVAR）において，landing zone延長・type Ⅱエンドリーク予防目的に，腹部分枝動脈への塞栓術が併施される^1）。塞栓物質としてはコイルやプラグが一般的であり，いずれも一定の治療成績を示している。一方で，コイル塞栓では複数本の使用が必要となることによるコスト増加^2）や，プラグ塞栓では再開通が問題となる場合があることが報告されている^3）。そのような現行デバイスの欠点を補う目的で新たな塞栓物質であるshape memory polymer（SMP）が開発され，SMPを用いた血管塞栓デバイスとしてIMPEDE塞栓プラグ（Shape Memory Medical社）が我が国では2020年2月より使用可能となった。

We report a case in which embolization was performed using parallel deployment of IMPEDE-FX. The IMPEDE embolization plug is a relatively new embolic device that utilizes a shape memory polymer（SMP）. It is expected to reduce the number of coils required for embolization and to improve long-term outcomes by suppressing recanalization. Deploying multiple IMPEDE-FX devices, which are derived from the IMPEDE embolization plug, in parallel, enables embolization of vessels larger than the foam diameter. This suggests that this technique may also be useful for achieving high-density embolization within short vascular segments.
