上眼瞼に認めたcrystal–storing histiocytosisの1例

A case of crystal–storing histiocytosis of upper eyelid

crystal–storing histiocytosis（CSH）は異常な結晶貪食を特徴とする組織球の増殖を認めるまれな病態であり，全身のあらゆる組織での発生が報告されている。背景疾患としてリンパ増殖性疾患や形質細胞疾患が高率に認められ，細胞質内結晶物は免疫グロブリン由来のものが多いとされている^1）2）。しかし，まれに非免疫グロブリン由来結晶の報告があり，一部では慢性炎症や自己免疫性疾患，感染症などと関連している可能性も示唆されている^2）3）。今回我々は，左眼瞼腫瘤として発見されたCSHの症例を経験したので若干の文献的考察を加えて報告する。

We report a case of crystal–storing histiocytosis（CSH）in upper eyelid. A man in his 70s presented with the chief complaint of an enlarged left eyelid mass, and contrast–enhanced MRI revealed an irregularly shaped mass extending along the upper eyelid. Partial resection was performed, and the pathological diagnosis of CSH was made. CSH is very rare pathological condition, and often associated with lymphoproliferative diseases or plasma cell dyscrasia.
