A 40-year-old man with untreated diabetes mellitus presented with left-side headache and left ophthalmalgia for three days. He was initially diagnosed with diabetic ketosis and admitted. Five days later from the onset, he developed a vesicular rash over the left side of his forehead. Magnetic resonance imaging showed enhancement of the left optic nerve sheath. Polymerase chain reaction test on his cerebrospinal fluid was positive for varicella-zoster virus. He was diagnosed with herpetic optic perineuritis. Optic perineuritis is a rare complication of herpes zoster ophthalmicus. We will report a case of herpes zoster ophthalmicus in which ophthalmalgia appeared prior to cingulate eruption, with some discussion of the literature.
