For Japanese women, breast cancer is the most common incident cancer and the 4th common cause of cancer deaths. The lifetime probability of developing breast cancer is one out of 9 women in Japan. The mortality has been still increasing. Japan has no benefit from screening mammography. To break through this stagnant situation, we must cause a paradigm shift of breast cancer screening in Japan. Artificial intelligence computer-aided detection synthesized mammograms(AI CAD SM)are the key to a next-generation breast cancer screening program in Japan. The use of the AI CAD SM+digital breast tomosynthesis(DBT)could allow for a more effective screening program with higher performance, especially in terms of an increase in the detection accuracy and a reduction of in the reading time as well as radiation dose, when compared to the digital mammograms(DM)+DBT. The AI CAD SM might be considered as standalone mammograms with higher performance compared to the DM.
