We develop an instantaneous wave-free ratio(CT-iFR)derived from dynamic coronary CT angiography combined low dose and boost scan. The present study investigates burdens of high-risk plaque on coronary circulation using CT-iFR and the diagnostic performance for detection of myocardial ischemia. CT-iFR for arteries with high-risk plaque was significantly lower than those for arteries with calcified plaque and non-plaque(0.71±0.17 vs. 0.94±0.20 vs. 0.93±0.21). CT-iFR for territories with ischemia was significantly smaller than those without(0.68±0.15 vs. 0.89±0.17). ROC analysis revealed the optimal cutoff of CT-iFR 0.73 for detecting myocardial ischemia with AUC of 0.88, a sensitivity of 56%, and a specificity of 99%. CT-iFR is associated with plaque characteristics, and is a non-invasive technique for detection of myocardial ischemia.
