腎部分切除後に生じた腎動静脈瘻に対し塞栓術を行った1例

A case of transarterial embolization for the treatment of arteriovenous fistula following partial nephrectomy

腎部分切除術は腎機能温存の観点から小径腎細胞癌に対する標準的な治療となっている。しかし，仮性動脈瘤や動静脈瘻といった血管性合併症も少数ではあるがみられる。発生頻度からは腎動静脈瘻よりは仮性動脈瘤のほうが多いとの報告が見受けられる。その治療は腎機能温存や低侵襲という観点からIVRでの経カテーテル治療が主体となっている。今回は腎部分切除術後に発生した腎動静脈瘻に対しマイクロコイルによる塞栓を行い，良好な塞栓効果を得た1例を経験した。手術手技と仮性動脈瘤・腎動静脈瘻も含め報告する。

Renal arteriovenous fistula is one of rare complication of partial nephrectomy but it is serious complication which can lead to severe hematuria, blood loss and hemorrhagic shock. We presented a case of renal arteriovenous fistula following partial nephrectomy which was successfully treated by renal artery embolization and briefly described the relationship beween surgical procedure and complications before and after embolization.
