Clinical PET and targeted radionuclide therapy are considered to be major roles of recent nuclear medicine practices in daily patient management. The concept of theranostics, which provides clinicians with valuable information at every step including patient selection, prediction and judgement of therapeutic efficacy and follow-up after treatment by means of diagnostic images using the same molecule as therapeutics, has been widely accepted. The situation in Japan is far behind the international development in all aspects of human resources, infrastructure and legislation. In order to secure benefits of patients, collaborative works among academia, industry, authority and patient groups are highly needed.
