著明な出血壊死をきたした膵神経内分泌腫瘍の1例

A case of pancreatic neuroendocrine neoplasm with massive hemorrhagic necrosis

膵神経内分泌腫瘍は，膵腫瘍の中では膵癌に次いで多くみられる疾患である。古典的には早期濃染像がみられる腫瘍とされているが，近年，非典型例の報告も多数認められる。今回我々は，出血壊死をきたし，非典型的な所見であった膵神経内分泌腫瘍の1例を報告する。

A 52 year-old man was pointed out abdominal mass by health checkup. CT showed the large tumor with heterogenous and prolonged enhancement pattern in pancreas head. Another tumor was a hilar lymph node adenopathy. MR showed low-iso intensity with high intensity spot on T1WI, and heterogenous high intensity with low intensity spot on T2WI. Distal pancreatectomy with lymph node dissection was done. Final diagnosis was pancreatic neuroendocrine neoplasm with massive hemorrhagic necrosis and a lymph node metastasis.
