Abstract 文献概要

要旨　患者は56歳，女性．主訴は嘔吐，左背部痛，入院時の臨床検査では特記すべき所見は認められない．ERCP所見では主膵管の圧排も認め，超音波，EUS，CT所見で膵尾部に充実性腫瘤を認め，内部と辺縁に嚢胞性変化を認めた．低緊張性十二指腸造影では十二指腸水平脚に圧排像を認め，壁外性の腫瘍と考えられた．血管造影検査で主に横行膵動脈の分枝から栄養される腫瘍濃染像を呈した．以上の画像所見から膵由来の腫瘍(島細胞腫またはSCT)と診断し，手術を施行した．腫瘍は膵とは容易に剥離可能で空腸原発の壁外性腫瘍と診断，空腸部分切除術を施行した．病理組織学的には筋層由来の十二指腸平滑筋肉腫であった．血管造影検査で横行膵動脈から栄養されていたため，膵腫瘍と鑑別が困難であった．

　A 56-year-old woman was admitted to our hospital with chief complaints of vomiting and back pain. Ultrasonographic examination showed a hypoechoic mass compressing the pancreatic body and tail. Abdominal CT scan revealed a central cystic cavity in the mass. Hypotonic duodenographic examination showed a smooth filling defect in the third portion of the duodenum. Superselective dorsal pancreatic arteriography demonstrated hypervascular mass with marked neovascular formation, dilated feeding arteries and tumor stain. The resected specimen showed a tumor, 8 cm in size, which arose from the duodenum. Histopathologic examination disclosed that it was Leiomyosarcoma.
