肥厚性硬膜炎は慢性炎症性疾患で，脳や脊髄の硬膜に局所的やびまん性の肥厚をきたす比較的まれな疾患である^1）。その中でも脊髄の肥厚性硬膜炎はかなりまれであり，日本における肥厚性硬膜炎の疫学調査によると頭蓋内が84.9％，脊柱管内8.8％，両方が4.4％と報告されている。原因としては原因不明の特発性（44％）が最も多いが，原因がわかったものとしては抗好中球細胞質抗体（antineutrophil cytoplastic antibody：ANCA）関連（34％），IgG4/multifocal fibrosclerosis関連（8.8％）が多いとされている^2）。
