乳癌センチネルリンパ節生検法の現状と課題

State of the art in sentinel lymph node biopsy for breast cancer patients

センチネルリンパ節（sentinel node：SN，見張りリンパ節）は，乳癌が最初に到達するリンパ節のことであり，このセンチネルリンパ節からリンパ節転移が生じるという仮説がSN理論である。すなわち，SNに転移がなければ，理論上は領域リンパ節転移の可能性はないとされる。乳癌における腋窩の手術に関して，SNの転移状況に応じて腋窩リンパ節郭清（axillary lymph node dissection：ALND）の有無を決定するセンチネルリンパ節生検（sentinel node biopsy：SNB）が広く普及している。本稿ではSNBの現状と課題について概説する。

Despite medical and radiation therapy has progressed remarkably, surgery is still one of the most important therapy for breast cancer. Axillary surgery for breast cancer as local therapy also inform clinicians the axillary lymph node status. In many patients with early breast cancer, sentinel lymph node biopsy has replaced axillary dissection. Accurate diagnosis and proper decision according to each cases are very important, because the inadequate omission of axillary dissection may influence the patient prognosis.
