変性疾患症例から学ぶ123I-MIBG心筋シンチグラフィと123I-FP CIT SPECTのpitfall

A case-based approach：pitfall of 123I-MIBG myocardial scintigraphy and 123I-FP-CIT SPECT in neurodegenerative disease

パーキンソン病（Parkinson’s disease：PD）に認められる動作緩慢，震えなどの所見をパーキンソニズムと呼ぶ。PD以外の神経変性疾患や薬剤，脳血管障害などでも認めることがあり，それらの疾患をまとめてパーキンソン症候群と称するが，その鑑別診断は時に容易ではないため，¹²³I-MIBG心筋シンチグラフィと¹²³I-FP-CIT SPECTが汎用されるようになった。しかし，PDなどの変性疾患の確定診断は，最終的には病理学的診断が必要である。このため，生前の臨床診断において，核医学検査の果たす役割は大きい。

Using ¹²³I-MIBG myocardial scintigraphy and ¹²³I-FP-CIT SPECT makes it possible to diagnose neurodegenerative disease more accurately. We proposed a diagnostic matrix based on radionuclide imaging for neurodegenerative disease. Since some cases show atypical imaging results, careful consideration of pitfalls is important for diagnosis. We present some cases of pitfalls in this manuscript.
