食道癌に対する内視鏡下手術—2モニター法

Thoracoscopic esophagectomy for esophageal cancer using 2 monitor method

　胸部食道癌に対する胸腔鏡下食道切除を2モニター法で行っている．通常開胸と同様の視野を術者，助手双方が得るために，それぞれの背側に配置したモニターの片方を天地左右逆転させている．これによりeye hand coordinationを保つことが可能となる．また，2モニター法では4か所に配置したポートのどこからカメラを挿入しても同じモニターを見ることができて操作が可能である．鉗子，カメラが鋭角に集中することがなく互いに干渉しにくいというメリットがある．現在行っている胸腔鏡下食道切除手技は小開胸を置かずに6ポートで行い，視野の展開には独自に工夫したendo HI retractorを用い，両側の反回神経周囲のリンパ節郭清の徹底化と両側迷走神経肺枝の温存，左右気管支動脈の温存に努めている．

We perform thoracoscopic esophagectomy to thoracoscopic esophageal cancer using the two monitor method. The 2 monitors are positioned behind each surgeon and first assistant. The views of the monitors are placed up side down and right side left in each side, facilitating the surgeon's and first assistant's eye hand coordination. This method allows us to continue and fulfill the operation seeing the same monitor whatever port is used for scope insertion.
