患者は，74歳，男性．下行結腸癌の診断で2004（平成16）年7月16日に，腹腔鏡下左半結腸切除，3群リンパ節郭清術を施行した．術前に，1か月で8kgの体重減少がみられた．術後2日目より食事を開始したが，嘔吐を繰り返した．上部消化管造影検査上，造影剤の十二指腸以下への流出は不良で，腹部CT像上，十二指腸水平脚が腹部大動脈と上腸間膜動脈により圧排，狭小化しており，上腸間膜動脈症候群と診断された．その後，保存的加療で通過障害は改善した．本症例は，術前からの体重減少の他，結腸切除時のリンパ節郭清や腸管吻合などにより，腸間膜根部が尾側へ牽引された結果生じたものと考えられるが，腹腔鏡下結腸切除後としては第1報目である．
A 74-year-old man was admitted to our hospital with a diagnosis of descending colon cancer. He had lost 8 kg in one month. He resumed oral nutrition 2 days after laparoscopic left hemicolectomy, but had recurrent vomit-ing after meals. Gastroduodenography showed obstruction of the third portion of the duodenum and abdominal computed tomography demonstrated compression between the superior mesenteric artery (SMA) and the ab-dominal aorta. The diagnosis was SMA syndrome, a possible complication of laparoscopic left hemicolectomy.
