腹腔鏡下大腸切除術の欧米と本邦における現況

Laparoscopic Colorectal Surgery for Cancer:Recent Trend in Japan and Western Countries

　腹腔鏡下大腸切除術は，低侵襲である点や整容的に優れている点で，従来の開腹手術より高く評価されている．しかし，癌の根治性が開腹手術と同等であるか否かの問題が，明確には解決されていない．また，技術的な習熟に時間を要することもあり，大腸癌の腹腔鏡下手術は腹腔鏡下胆嚢摘出術ほど広まっていない．本術式が施行されるようになってから，欧米では11年，本邦では9年が経過しているが，その普及の速度は遅いのが現状である．しかし，欧米においても本邦においても，本術式は徐々に確立した術式として定着しつつあり，今後，大腸の手術において欠くことのできない重要な位置を占めていくことが予想される．本術式の発展過程，適応の変遷，技術的問題点，遠隔成績などについて述べ，本邦および欧米における現状について解説した．

The advantages of laparoscopic colectomy are less invasiveness and better cosmetics because of the small sur-gical wound. However, it is still unclear whether laparoscopic colectomy for colorectal cancer is as curative as open colectomy. The technical difficulty is another problem. Despite of such problems, the availability of this pro-cedure is gradually increasing, and in Western countries as well as in Japan, this procedure is becoming an es-tablished procedure as a treatment of colorectal carcinoma.
