整容性の評価

Cosmetic assessment

　乳腺内視鏡手術の目的は整容性の向上である．その整容性の評価方法は，従来の4段階法の他，乳房温存療法ガイドラインによる評価法があった．より客観的な評価には，BRA法，沢井班研究報告による整容性評価法と，われわれが考案した5項目4段階ABNSW法がある．ABNSWは，乳房の位置，形状，乳頭の変形，皮膚の状態，瘢痕について4段階に評価し，合計点で総合判定する．乳腺内視鏡手術後には90％が11点以上となり，この手術の整容性のよさが示された．また，再建方法として，乳腺授動法，側胸部脂肪織充塡法，合成繊維網充塡法（FMS法）があり，充塡法が整容性で勝っていた．乳腺切除量が多い場合には充塡による再建が必要である．

The purpose of the endoscopic breast surgery is to get a better cosmetic outcome. The assessment of the cos-metic results is performed by the ordinary 4 step method and the method described in the Japanese guideline for breast conserving therapy. The more objective quantitative methods are BRA (breast retraction assess-ment), a method researched by Dr. Sawai, and 5-item-4-step method (ABNSW) which we have designed.
