早期胃癌に対する腹腔鏡補助下幽門側胃切除術（D1＋α）

Laparoscopy-assisted distal gastrectomy with D1+α lymph node dissection for early gastric cancer

　胃癌に対する低侵襲手術として腹腔鏡補助下幽門側胃切除術（LADG）が導入され，10年以上が経過した．新しい手術機器の開発と手術手技の向上により，LADGは安全で有用な手術として確立しつつある．早期胃癌に対する治療の選択肢として，多くの施設がこの手技を取り入れて良好な成績を収めている一方で，煩雑な手術操作から導入を躊躇する施設もまだ多く見受けられる．腹腔鏡下手術では，視野の確保・保持，器具の操作性・自由度に制限があるため，安全で正確なリンパ節郭清を行うにはさまざまな工夫が必要となる．

　本稿では，LADGの基本であるD1＋α〔No7（下部胃癌ではNO8aも郭清）〕郭清の手術手技とピットフォールについて解説した．

With the development of related instruments and techniques, laparoscopy-assisted distal gastrectomy (LADG) has come to be applied to the treatment of early gastric cancer as a minimally invasive surgery. However, it has not been widely accepted because of its technical difficulties. Since it is difficult to control the instruments and to acquire sufficient view, the same contrivance are necessary for LADG to perform safely and securely. In this report, we will describe the LADG with regional lymph nodes dissection (D1+alpha).
