Recently,less invasive surgery for early gastric cancer is advocated. We performed laparoscopic distal gastrectomy with D2 lymphadenectomy by the hand-assisted method for 20 patients of gastric carcinomas located at middle or lower part of the stomach.The lesions were all limited to the muscularis propria or less in tumor depth and with no obvious lymph node metastases in preoperative radiological and endoscopic examinations.A transverse skin incision about 6 cm length in the right lower quadrant was available to insert the surgion's left hand into the abdominal cavity, and five other ports were put.
