胃癌に対する腹腔鏡下幽門側胃切除術およびD2リンパ節郭清

Laparoscopic Distal Gastrectomy with D2 Lymphadenectomy for Gastric Cancer

　近年，早期胃癌に対する縮小手術が提唱されている．一方，腹腔鏡下手術がその低侵襲性のためQOLを高める手技として注目されている．今回われわれは，術前診断で深達度MPまでで，明らかなリンパ節転移のみられない中下部胃癌に対して，幽門側胃切除術およびD₂リンパ節郭清をhand—assisted methodで腹腔鏡下に20例に施行し良好な結果を得たので報告する．右下腹部に左手が入る程度の約6cmの横切開をおき，トロッカーを5か所に挿入する．原則として通常の手順どおりに操作を進め，No．1,3,4d,4sb,5,6,7,8a,9,11pリンパ節の郭清を行う．症例によりNo．12a,14vリンパ節の郭清を追加した．胃十二指腸吻合はタバコ縫合器とサーキュラーステイプラーを用いて体内で行った．術後の疼痛は軽度で，早期の離床，経口摂取開始および退院が可能であった．

Recently,less invasive surgery for early gastric cancer is advocated. We performed laparoscopic distal gastrectomy with D₂ lymphadenectomy by the hand-assisted method for 20 patients of gastric carcinomas located at middle or lower part of the stomach.The lesions were all limited to the muscularis propria or less in tumor depth and with no obvious lymph node metastases in preoperative radiological and endoscopic examinations.A transverse skin incision about 6 cm length in the right lower quadrant was available to insert the surgion's left hand into the abdominal cavity, and five other ports were put.
