腹腔鏡補助下幽門側胃切除術における手技の定型化—アプローチポイントと郭清確認因子

Technical Standardization of Laparoscopy-assisted Distal Gastrectomy: How to Approach and Dissect Lymph Nodes Stations?

　腹腔鏡補助下幽門側胃切除術は創痛が少なく，術後の回復も早いが，癌の治療としてはリンパ節郭清が不十分になるのではないかという批判を受けており，郭清手技の難しさも指摘されている．われわれは開腹術と同等のD_1＋α，あるいはD₂郭清を腹腔鏡下に確実に施行するために，胃周囲の血管を剥離・露出しその根部で切離することで，その領域のリンパ節を完全に郭清したと表記することにしている．そのために，リンパ節領域ごとに郭清確認因子を設定し，さらにその郭清確認因子に達するアプローチ法の確立に努力してきた．これにより，開腹術と同等の郭清であることを確認することが可能となり，郭清精度の客観的な評価が可能になると考える．

Laparoscopy-assisted distal gastrectomy has been criticized for its difficulty and imcomplete dissection of perigastric lymph nodes, despite its merits such as less wound pain, and early resumption of normal function. We have been trying to perform D_1+α, D₂ dissection that ar equivalent to open surgery. We defined lymph node dissection as complete when the perigastric vessels were exposed and divided at their roots. We have also stan-dardized how to approach the roots of perigastric vessels. In this way, we can evaluate objectively whether the lymph node dissection is complete or not.
