胸部交感神経遮断術の適応と問題点

The indications and the problems in endoscopic thoracic sympathectomy

　胸腔鏡下交感神経切除術の適応症には，手掌多汗症，末梢血行障害，神経因性疼痛，赤面恐怖と顔面多汗症などがある．近年になって，この手術を行う施設が増加したため，不十分な説明と同意のために副作用である代償性発汗で問題となる事例もみられるようになった．また，代償性発汗は起こらないとの主張の下で不完全な手術法を行う施設もある．どのような術式でも，その副作用である代償性発汗を防止することはできない．そのため，手術から得られる利益と術後に生じる代償性発汗の不利益を1人1人の患者に対して十分な時間の個人面接を行って説明し，手術の可否を決定する必要がある．赤面恐怖の患者では，精神科医もしくは臨床心理士の面接も必要である．適応の選択は慎重に行うべきである．

Palm hyperhidrosis, peripheral vascular disease, neuropathic pain syndrome, facial blushing and facial hyperhydrosis are indications for endoscopic thoracic sympathecotmy (ETS). Recently, as the number of hospi-tals which practice ETS incrcases, many troubles relating to compensatory hyperhidrosis seem to come into the open because of insufficient informed consent. There are institutims thet perform incomplete sympathetic trunk resection and aduocate that compensatory hyperhidrosis will not ocuur after surgery. But no matter what op-eration or technigue is used, compensatory hyperhidrosis cannot be completely prevented.
