手掌多汗症に対する胸腔鏡下手術のPros and Cons

Pros and cons of thoracoscopic surgery for primary palmar hyperhidrosis

　わが国でも胸腔鏡下手術が種々の胸部疾患にも適用され普及してきた．手掌多汗症に対する胸腔鏡下胸部交感神経遮断術が多くの施設で行われるようになった．さらに，その手技も各施設で異なり，現況では片側手術，両側手術などの考え方があり手技が確立されていない．多汗症に対する絶対的適応はなく，十分なインフォームド・コンセントを得たうえで，本人の強い悩み・希望に応えるものである．術後は代償性発汗の出現に納得できず，新たな深刻な悩みになることがあり，その適応には十分な選別が重要と考える．本稿では手掌多汗症に対する手術の適応，手技，合併症，その対策などについて述べるととともに，本手技の注意点について述べた．

Video-assisted Thoracoscopic Surgery (VATS) has been used for several thoracic disease in Japan. With the rapid progression of video-assisted endoscopic technique, a thoracoscopic sympathectomy or sympathicotomy has become the procedure for treating essential hyperhidrosis in many institutions. VATS sympathectomy can be performed by a number of different surgical approaches, such as bilateral or ipsilateral procedures, therefore, the procedures for primary palmar hyperhidrosis have not been established at the present time.
