胸部交感神経切除術の工夫と評価

Device and evaluation of thoracic sympathectomy

　1992年8月から2003年9月までに，手掌および腋窩多汗症の393例（男性154例，女性239例）に対して胸腔鏡下胸部交感神経切除術を施行した．手術時の年齢は11歳〜65歳（平均29.6歳）であった．手掌多汗症が350例，腋窩多汗症が40例，顔面多汗症が3例であった．術後の再発例はなく，手技の成功率は100％であった．主な術後合併症は血胸2例（0.5％），気胸2例（0.5％），胸水2例（0.5％）などでホルネル症候群はなかった．代償性発汗は術後にほとんどの患者で躯幹，腹部，大腿部に認められた．術後のアンケート調査では295例が回答し，そのなかで278例（94.2％）が満足と回答した．

Three hundred and ninety-three patients (154 male, 239 female) with palmar and axillary hyperhidrosis un-derwent thoracoscopic sympathectomy in our hospital between August 1992 and September 2003. The perioperative age was 11-65 (mean 29.6 years old). There were 350 patients with palmar hyperhidrosis, 40 with axillary hyperhidrosis and 3 with craniofacial hyperhidrosis. No recurrence occurred postoperatively, and the success rate was 100%.
