腹腔鏡下膵体尾脾切除術を施行した膵Lymphoepithelial Cystの1例—Cherry Dissector®による視野展開

Laparoscopic Distal Pancreatectomy for a Lymphoepithelial Cyst of the Pancreas Using Cherry Dissector® as a Retractor

　近年，内視鏡下手術の適応は拡大しているが，膵臓はその解剖学的位置や手術対象となる疾患内容から，現在のところ膵臓の切除術が内視鏡下手術の対象となることは少ない．今回，われわれは膵臓原発のlymphoepithelial cystという稀な嚢胞性腫瘍に対して腹腔鏡下での膵体尾脾切除術を施行した．われわれは以前から腹腔鏡下脾臓摘出術の視野展開におけるCherry Dissector^®（ジョンソン・エンド・ジョンソン社）の有用性を報告してきたが，本稿では，Cherry Dissector^®を用いた視野展開による膵尾部の嚢胞性病変に対しての膵体尾脾切除術を報告するとともに，腹腔鏡下膵体尾切除術の適応疾患について考察を加えた．

Laparoscopic distal pancreatectomy is not a common procedure in pancreatic surgery. We reported here a patient who suffered pancreatic lymphoepithelial cyst, which is a rare pancreatic neoplasm. We resected this tumor because of the elevation of serum CA 19-9 and the high level of tumor markers (CEA, CA 19-9, CA 125, Span 1) in the cystic fluid of this tumor. We emphasize the potential usefulness of the Cherry Dissectors as a retractor in the parenchymatous organ.
