腹腔鏡下胆嚢摘出術後ポートサイトヘルニアの3例

Three Cases of Incisional Hernia at the Trocar Site after Laparoscopic Cholecystectomy

　腹腔鏡下胆嚢摘出術後の合併症の1つであるポートサイトヘルニアへの対応策として，臍上部腹直筋鞘の縫合閉鎖が推奨されている．われわれは臍上部腹直筋鞘閉鎖症例において，3例のポートサイトヘルニアを経験したので報告する．ポートサイトヘルニアは欠損部分が小さいため嵌頓症状を起こしやすいと言われ，われわれの3例中1例も急性期Richter型ヘルニアを合併し，小腸切除を必要とした．また1例は直視下腹直筋鞘縫合閉鎖ののち，1年を経過してポートサイトヘルニアが発生した．臍周囲ポート挿入創の閉鎖に際しては確実な腹壁閉鎖が必要であるが，通常の開腹手術と同様に晩期の腹壁瘢痕ヘルニア併発にも注意する必要がある．

Direct closure of the umbilical fascia is recently recommended for the prevention of incisional hernia at the umbilical trocar site. This incisional hernia is one of the postoperative complications for laparoscopic cholecystectomy. We reported herein three cases of incisional hernia at the umbilical trocar site despite the unbilical fascia closure. It has been reported that the incisional hernia at the trocar site may easily result in in-carceration of Richter hernia.
