外傷性膵仮性嚢胞に対して吊り上げ併用下に腹腔鏡下胃嚢胞吻合術を施行した1例

Laparoscopic Cystgastrostomy using a Wall-lifting Procedure for a Pancreatic Pseudocyst: A Case Report

　外傷性膵仮性嚢胞に対し，吊り上げ併用下に腹腔鏡下胃嚢胞吻合術を施行し得た1例を経験した．症例は54歳，男性．ハンドル外傷にて膵嚢胞を形成し，上腹部痛が持続した．入院時，上腹部正中に手拳大の腫瘤を触知した．CT上6×7×8cmの嚢胞を認め，胃透視では胃幽門部から十二指腸に圧排所見があった．ERPにて嚢胞と膵管との交通は認められなかった．腹腔鏡下観察時，胃後壁と膵嚢胞との癒着を認めず，手術は内瘻術として腹腔鏡下胃嚢胞吻合術を施行した．術後，嚢胞は消失，症状は改善し術後2週目に退院した．現在まで再発合併症は認めていない．吊り上げ併用下の腹腔鏡下胃嚢胞吻合術は小切開創で済むため，術後創痛が軽度で患者の早期社会復帰が可能となり，術視野も良好であり有効であった．

The standard treatment for either symptomatic or large pancreastic pseudocysts (PC) is internal drainage from the cyst to the stomach or jejunum. We herein report the case of a successful laparoscopic cystgastrostomy using a wall-lifting procedure for PC. A 54-year old, male carpenter, suffered a traffic accident. He was thereaf-ter referred to our hospital due to persistent epigastric pain related to a PC. The patient's blood chemistry data were within the normal limits. The PC was shown to measure 6×7×8 cm in size based on the computed tomography (CT) findings.
