左側胆嚢胆石症に対する腹腔鏡下胆嚢摘出術の経験

A Case of Left-sided Gallbladder Treated with Laparoscopic Cholecystectomy

　左側胆嚢結石症に対する腹腔鏡下胆嚢摘出術を経験した．左側胆嚢は腹部超音波検査で術前診断が可能であった．本症例では右側門脈臍部が形成されており，左側胆嚢の発生機序として肝円索位置の異常による相対的左側胆嚢が考えられた．左側胆嚢を腹腔鏡下に摘除する場合には，視野を確保するために肝円索を体外から牽引すること，カロー三角部を展開させるために肝床部切離を先行して行うこと，また，操作を容易にするためには術者用ポートの位置を肝円索の左側とすること，などが肝要と考えられた．

We report a case of cholelithiasis in a patient with left-sided gallbladder that was treated with laparoscopic cholecystectomy. Ultrasonography is useful for preoperative diagnosis of the left-sided gallbladder. This patient had an anomalous right sided round ligament, which caused the gallbladder at the normal sight to be placed on the left side of the round ligament.
