右側肝円索・左側胆囊結石症に対して単孔式腹腔鏡下胆囊摘出術を施行した1例

Single port laparoscopic cholecystectomy for patient with right-sided round ligament/left-sided gallbladder : a case report

◆要旨：患者は71歳，女性．心窩部違和感を自覚し当院を受診した．CTにて胆囊結石症と診断し，単孔式腹腔鏡下胆囊摘出術を施行した．下腹部正中の卵管結紮術後の手術瘢痕に沿って2.5cmの皮切を置き，SILS^TMポートを挿入した．胆囊は肝円索の左側に位置し，肝円索は門脈右前区域枝に付着する右側肝円索であった．胆囊が通常より左側に位置していたため，腹腔鏡や鉗子同士の干渉が多かったが，SILS^TMポートを回転させてトロッカーの位置を変えることで操作性は改善した．胆管の走行異常はなく，通常通り胆囊摘出術を行い，術中・術後合併症もなく経過は良好であった．右側肝円索症例に対しても，単孔式腹腔鏡下胆囊摘出術は有用であると考えられた．

　We report a case of cholecystolithiasis with a right-sided round ligament treated by single-port laparoscopic cholecystectomy. A 71-year-old woman was referred to our hospital because of discomfort in the upper abdomen. The patient had undergone percutaneous transhepatic gallbladder aspiration for acute cholecystitis 5 years before. Abdominal CT revealed gallbladder stones and a right-sided round ligament. We performed single-port laparoscopic cholecystectomy successfully without conversion to conventional laparoscopy. The postoperative course was uneventful. We describe the case and discuss the key procedures for safe single-port laparoscopic cholecystectomy for a patient with right-sided round ligament from our experience.
