胸腔鏡下左内胸動脈剥離を行い，左開胸下に回旋枝領域に対しMIDCABを行った1例

A Case of Video-assisted LITA Harvest and Lateral MIDCAB to the Left Circumflex Artery

　症例は44歳，男性．主訴は労作時の胸痛．冠動脈造影検査で＃11近位部に95％の狭窄を認め，外科に紹介となった．左内胸動脈（LITA）を＃13に吻合できるよう近位部まで長く剥離するために，胸腔鏡下に剥離を行い，第2肋骨上縁付近までLITAを剥離した．その後，第5肋間で開胸，回旋枝（＃13）を露出し，心拍動下に吻合を行った．術後経過は順調で，グラフト造影検査でも良好に開存していた．胸腔鏡下左内胸動脈剥離により＃13へのMIDCABが可能であり，回旋枝領域のintervention困難例に対する1つの選択肢となり得ると考えられたので報告した．

We report a case of video-assisted LITA harvest and lateral MIDCAB to the left circumflex artery. A 44-year -old male patient suffered from anginal chest pain. Coronary angiography revealed severe stenosis of the cir-cumflex artery (#11) . Since intervention was difficult due to proximal lesion, CABG was recommended. To dis-sect the LITA long enough to reach the circumflex artery as a pedicle, thoracoscopic procedures and video-assisted harvest were performed.
