経腟的アプローチを併用した腹腔鏡下胃原発gastrointestinal stromal tumor摘出術

Laparoscopic Extirpation of Gastrointestinal Stromal Tumor (GIST) of the Stomach Assisted by a Transvaginal Approach

　胃原発gastrointestinal stromal tumor（GIST）の2症例に対し，経腟的アプローチを併用した腹腔鏡下摘出術を施行した．症例1：65歳，女性．胃体上部後壁に径50mm大の腫瘍が認められた．Endo GIAを用いた胃部分切除を行い，経膣的に摘出した．症例2：69歳，女性．胃体下部前壁に径40mm大の腫瘍が認められた．気腹下に後腟円蓋から挿入した把持鉗子を術野展開の補助として腫瘍核出術を行い，経膣的に摘出した，本手技の主目的は腹部切開創を延長することなく大きな腫瘍を摘出することであるが，操作用鉗子の挿入経路として用いることも可能であり，腹腔鏡下手術の利点を活かせる1つの工夫として広く応用が可能であると考えられた．

We employed a transvaginal approach in laparoscopic surgery for extirpation of gastrointestinal stromal tumor (GIST) of the stomach. The first case, a 65-year-old woman, underwent laparoscopic partial resection of the stomach with an endoscopic linear stapler device for a 50 mm-large GIST from the posterior wall of the upper stomach. The resected tumor was extracted through the posterior fornix of the vagina without extending the abdominal incision. The second case, a 69-year-old woman, underwent laparoscopic surgery assisted by a transvaginal approach for a 40mm-large GIST on the anterior wall of the lower stomach.
