早期大腸癌における内視鏡的摘除と腹腔鏡下手術の接点—拡大内視鏡によるpit pattern診断の有用性

Endoscopic Resection and Laparoscopic Surgery in Early Colorectal Cancer:Efficacy of Magnifying Endoscopic Diagnosis of Pit Patterns

　今日の早期大腸癌に対する治療体系は，polypectomyと内視鏡的粘膜切除術（endoscopic mucosal resection：EMR）に代表される内視鏡的摘除と，今や腹腔鏡下手術が中心となった感のある外科的切除から構成される．一方，早期大腸癌においては，深達度診断を主とした正確な診断が，治療を選択するうえで不可欠である．深達度診断については，超音波内視鏡（EUS），通常内視鏡検査によるnon-lifting signのほかに，拡大観察によるpit pattern診断により，その精度が向上してきている．特に，拡大内視鏡を用いた病変の拡大観察は，従来の内視鏡診断学にミクロ的診断学を加えることとなり，内視鏡診断を病理組織診断に近づけたと考える．拡大観察を用いたpit pattern診断という精度の高い診断により，早期大腸癌に対する適正な治療の選択が可能である．

There are two major streams in the current treatment of early colorectal cancer; endoscopic resection repre-sented by polypectomy and endoscopic mucosal resection (EMR). and laparoscopic assisted surgery.

In connection with that. it has become indispensable to diagnose accurately the depth of invasion, which sup-posed to be the key factor in the choice of the treatments in early stage of colorectal cancer.
