腹腔鏡下大腸切除術における新しい創縁保護器

New Wound Protector for Laparoscopic Colorectal Surgery

　フレキシブルリングとラテックス膜の張力で腹壁小切開創の空間を保持可能な創縁保護器ラッププロテクターを考案し，腹腔鏡下大腸切除術に用いた．ラッププロテクターはプラスチックシートの創縁保護材に比べて開創空間が広く，創の圧迫による腸管虚血の予防が可能であった．腹腔鏡下大腸切除術においては，癌の創再発や創感染の予防に腸管を摘出する小切開創の創縁保護を行うことが重要であるが，ラッププロテクターは十分な開創空間が得られることから，特に小さな開腹創や肥満症例で有用と考えられた．

New wound protector, a lap protector, which could maintain a small incisional space using the tension between the flexible ling and the latex membrane was used for laparoscopic colon surgery. Because operation space using lap protector was larger than polyethylene seat wound protector, the prevention of colon ischemia was easier. In order to prevent wound infection and cancer recurrence at wound site, it is important to protect small laparotomy incision in laparoscopic colorectal surgery. Lap protector is useful in particularly small laparotomy incision and obese patient because sufficient space is provided.
