食道癌の3例に片手補助腹腔鏡下の経食道裂孔的な食道抜去術を行った.上腹部正中に片手挿入のための約6cmの小切開をおき,左腹部に12mmのトロッカーを3本,留置した.手術手順は,まず脾臓の下極の高さで,LCSを用いて左胃大網動静脈,短胃動静脈を処理し,胃脾間膜を完全に遊離した,ついで,LCSを用いて食道裂孔から胃の小轡側への剥離を行い,左胃動静脈を切断した.縦隔内の食道周囲の結合織をLCSで切離し,食道を腹腔内に引き出した.授動された食道・胃を体外に引き出し,右側大網の処理を行った.胃管作成後,後縦隔経路にて頸部食道・胃管吻合を行った.3例の平均手術時間は210分,平均出血量は100mlであった.
We performed Hand-assisted laparoscopic transhiatal esophagectomy in 3 patients with esophageal cancer. A small incision of about 6cm at the upper median abdomen was made for the insertion of the left hand, and 3 trocars of 12mm in diameter were placed in the left upper abdomen. For the surgical procedure, left epiploic vessels and short gastric vessels were first dissected using Laparosopic Coagulating Shears (LCS) at a height nearly equal to the inferior edge of the spleen. As a result, the stomach was completely free from the gastrosplenic ligament.
