腹腔鏡補助下手術を施行した腸重積合併虫垂粘液嚢腫の1例

A Case of Mucocele of the Vermiform Appendix Associated with Intussusception Assisted by Laparoscopic Surgery

　虫垂粘液嚢腫は比較的稀な疾患であり，術前診断は困難とされている．今回われわれは，腹腔鏡にて本症および同病変による腸重積を診断し，腹腔鏡補助下回盲部切除術を行った1例を経験した．症例は67歳，女性．主訴は右下腹部痛．種々の検査にて本症を疑い，腹腔鏡にて観察したところ，虫垂は嚢胞状に腫大し，根部は上行結腸に陥入していた．以上にて本症による腸重積と診断し，腹腔鏡補助下回盲部切除術を施行した．病理組織学的診断はsimple mucoceleであった．本症に対して腹腔鏡下手術が行われた報告はこれまでなく，腹腔鏡下手術が有用である疾患の1つと思われたので，若干の文献的考察を加え報告する．

Mococele of the vermiform appendix is a relatively rare disease, and preoperative diagnosis for it is thought to be difficult. We experienced a case of intussusception due to mucocele of the vermiform appendix, and performed an laparoscopically assisted ileocecal resection. A 67-year-female complained of right lower abdominal pain and underwent diagnostic laparoscopic examination. It revealed that the appendix was cystically swollen, and the base was invaginated into the ascending colon.
