乳房温存手術における内視鏡下腋窩郭清術

Endoscopic Lymph Node Dissection in Breast Conserving Therapy

　1997年以降，乳房温存手術における腋窩郭清を従来の腋窩郭清の精度を下げることなく，より低侵襲で行えるかを検討する目的で，内視鏡下郭清術を施行してきた．創は腋窩には入れず，乳房外側上方で下着に隠れる部分に放射状切開を置き，光学視管と一体化させたレトラクターで胸筋を挙上しつつレベルIIIまでの郭清を行う．En-blockな郭清を行うため，創を3cm以下には縮小できなかったが，従来の創よりも3分の1以下に縮小でき，従来法と同等以上の郭清が可能であった．合併症もなく，整容性をはじめとした患者の満足度は良好であった．観察期間は最長3年3か月で遠隔転移による再発を1例に認めたが，乳房内や局所には再発を認めていない．

Endoscopic surgery has recently been applied for several breast surgeries.In breast conserving therapy. al-though determining the level of axillary node involvement is the single best predictive factor and is essential for planning postsurgical therapy, recently, avoiding axillary surgery and sentinel lymphadenectomy have been ad-vocated to decrease the morbidity associated with total axillary dissection.
