頸部皮膚吊り上げ法（VANS法）による甲状腺微小乳頭癌に対する内視鏡下甲状腺片葉切除術とリンパ節（D1）郭清術

Endoscopic Hemithyroidectomy with Central Neck Dissection for Micro-papillary Carcinoma of the Thyroid Using Totally Gasless Anterior Neck Skin Lifting Method (VANS Method)

　甲状腺や上皮小体に対する内視鏡下手術が普及するなか，われわれは現在までに130例以上の良性の結節性甲状腺腫，上皮小体腫瘍に対して内視鏡下手術（video-assisted neck surgery：VANS法）を施行し，整容上良好な結果を得ている．開襟の衣類で被われる患側の鎖骨下前胸壁に約3cmの皮膚切開と，側頸部に光学視管用の5mmの皮膚切開をおき，working spaceは1.2mmのキルシュナー鋼線2本で前頸部皮膚を前方に吊り上げ確保している．手術操作は鎖骨下の主切開創から超音波凝固切開装置と鉗子を用いて行う．今回，適応を甲状腺微小乳頭癌にまで拡大し，甲状腺片葉切除術とD₁リンパ節郭清術を安全かつ合併症なく施行し得た．本術式は通常の手術と遜色なく原発巣の切除やリンパ節郭清を施行することが可能であり，また整容上も有用性があると考えられた，手術時間は通常手術に比べかかるが，経験症例を積むことによって短縮可能と考えている．甲状腺片葉切除術とD₁リンパ節郭清術を通常手術と同様に施行可能な本術式は，甲状腺微小乳頭癌に対する手術術式の1つとなると思われた．

Endoscopic thyroid and parathyroid surgery has rapidly developed in the last few years. We have treated more than 130 cases of benign thyroid and parathyroid tumors using a totally gasless anterior neck skin lifting method (Video-Assisted Neck Surgery: VANS method). This surgical procedure is certainly beneficial for the patients from a cosmetic viewpoint. An approximately 3cm incision was made on the tumor side of the chest wall covered by open-neck clothing and a 0.5cm incision was made in the lateral neck on the tumor side for the endoscope.
