　内視鏡下手術が一般化したなかで，甲状腺，上皮小体腫瘍に対する本術式の報告は少ない．われわれは，現在まで70例以上の良性甲状腺，上皮小体腫瘍に対し美容上の観点を重視した内視鏡下摘出術（video-assisted neck surgery：VANS）を行っている．開襟衣類で被われる鎖骨下前胸壁に小切開を加え，working spaceは，前頸部皮下を十分に剥離したのちKirschner wire 2本で前方へ牽引する吊り上げ法を応用している．腫瘍摘出には主に超音波凝固切開装置を使用，合併症もなく手術操作が行われ，いずれの症例も美容上，きわめて満足すべき結果が得られている．手術時間，出血量は経験とともに改善されており，本術式は簡便かつ安全で，今後広く応用され得る術式の1つであろう．

Reports of endoscopic thyroid surgery are extremely limited despite recent expansion of the endoscopic surgery in the surgical field. We have experienced more than 70 cases of the thyroid and parathyroid tumors using the gasless anterior neck lifting method (Video-Assisted Neck Surgery : VANS) while stressing the importance of the cosmetic aspect. A main incision depending upon a tumor size was made on the tumor side of the chest wall, 1.5-2.0 cm below the clavicle, where the operative scar was completely covered by open -neck clothing.
