開腹既往例に対する腹腔鏡下胆嚢摘出術

Laparoscopic Cholecystectomy in Patients with Previous Abdominal Surgery

　開腹既往歴を有する患者に対する腹腔鏡下胆嚢摘出術（以下，LC）を紹介する．当院での9年間のLC症例2,692例中，上腹部開腹既往例は104例，下腹部開腹既往例は606例であった．上腹部開腹既往例ではLCからの開腹移行率が22％と有意に高率で，手術時間，術後在院日数とも延長した．一方，下腹部手術既往例のLCは特に問題なく施行でき，開腹歴を有しない群と有意差はなく，デイサージェリーも可能と思われた．上腹部開腹既往例に対するLCでは十分な腹腔内空間と胆嚢周囲の良好な術野を得るための慎重な癒着剥離操作が重要となる．途中で腸管損傷の危険があると判断した場合は，躊躇することなく開腹へ移行することが大切である．

Of 2,692 patients who underwent laparoscopic cholecystectomy (LC), 104 patients had previous upper abdominal surgery (UAS) and 606 patients had lower abdominal surgery (LAS). The conversion ratio from LC to open surgery in the patients with UAS was 22%, which was significantly higher than that in the patients without previous laparotomy. Previous UAS resulted in longer operation time and postoperative hospital stay for LC. On the other hand, LC in the patients with previous LAS had no significant difference with that in the patients without previous laparotomy.
