腹腔鏡下総胆管結石治療の現況と問題点—長崎内視鏡外科研究会アンケート調査

Management of Bile Duct Stones in the Laparoscopic Era:A Questionnaire Survey by the Committee of Nagasaki Endoscopic Surgery

　長崎内視鏡外科研究会関連施設（以下，関連施設）における総胆管結石に対する腹腔鏡下手術についてアンケート調査を行った．関連施設において，腹腔鏡下総胆管切石術は1992年から始まっているものの，回答が得られた施設（31施設）のうち11施設で行われているにすぎず，症例数も5例以下が多く，20例を超えたのは2施設であった．腹腔鏡下総胆管切石術の合併症は11％で，胆汁漏出，結石の遺残再発，十二指腸穿孔，胆管狭窄などであった．死亡例はなかった．総胆管結石に対し，内視鏡的経乳頭的切石術と，開腹—総胆管切開—Tチューブドレナージが現時点では最もよく選ばれる選択肢であったが，腹腔鏡下総胆管切石術の導入を予定している施設は60％であった．

To review the management of gallstones in the common bile duct during the past 7 years, questionnaires were mailed to 75 institutes that are affiliated with the Committee of Nagasaki Endoscopic Surgery. Since 1994, laparoscopic choledocholithotomy was performed in 11 of 31 institutes (34% of the respondents) that responded to the questionnaries. Only two institutes experienced more than 20 laparoscopic choledochotomy, while many others had less than five. Complications of laparoscopic choledocholithotomy have occurred in 11%. e.g. bile leakage, residual stones.
