腹腔鏡下総胆管切石術のコツ

Knacks of Laparoscopic Choledochotomy

　われわれは，腹腔鏡下総胆管切石術における縫合操作を容易に行うために，正中上部のポートの位置を腹腔鏡下胆嚢摘出術のときよりも下方の，臍と剣状突起との中点においている．これにより総胆管の縫合操作が速く確実に行えるようになり，手術時間は平均135分間と短縮され，胆汁漏などの合併症もほとんど認められなくなった．腹腔鏡下総胆管切石術を容易に行うために，われわれが留意しているコツについて述べたい．

We have modified the position of the upper midline port in laparoscopic choledochotomy to midpoint between xyphoid process and umbilicus., which is a lower position than that in laparoscopic cholecys-tectomy. At this position, we can suture the common bile duct easier, the operation time became shorter (medium 135 minutes), and no long term bile leakage have been experienced.
